Why Wrist Pain Diagnosis Matters

Wrist pain is one of the most commonly self-misdiagnosed musculoskeletal complaints. Patients assume carpal tunnel when they have cubital tunnel. They treat tendinitis when they have a ganglion cyst. They assume a sprain when they have a scaphoid fracture. Getting the right diagnosis before starting treatment saves months of wasted effort.

The Most Common Causes of Wrist Pain

Carpal Tunnel Syndrome

Median nerve compression within the carpal tunnel produces numbness and tingling in the thumb, index, middle, and half the ring finger, pinch grip weakness, and nocturnal pain. Mild to moderate cases respond well to conservative management β€” wrist splinting, neural mobilization, and addressing contributing posture and loading factors.

De Quervain's Tenosynovitis

Inflammation of the first dorsal compartment tendons causes pain at the base of the thumb. The Finkelstein test reproduces it. Common in new parents, office workers, and athletes. Responds well to Graston technique and activity modification.

Wrist Tendinopathy

Overuse of wrist flexors or extensors in CrossFit athletes (hook grip, bar work), rock climbers, and manual workers. Treatment: load management, Graston technique, progressive eccentric loading.

TFCC Injuries

The triangular fibrocartilage complex stabilizes the ulnar wrist. Tears cause ulnar-sided wrist pain with weight-bearing and forearm rotation. A frequently missed diagnosis.

Ganglion Cysts

Fluid-filled cysts from joint capsules or tendon sheaths. Most common on the dorsal wrist. May be asymptomatic or cause aching pressure-related pain. Many resolve spontaneously.

Scaphoid Fractures

The most commonly fractured carpal bone, from a fall on an outstretched hand. Classic presentation: anatomical snuffbox tenderness after a FOOSH injury. Critical to diagnose promptly β€” missed scaphoid fractures can progress to avascular necrosis.

Important: Any wrist injury following a fall with anatomical snuffbox tenderness should be evaluated urgently and treated as a scaphoid fracture until proven otherwise. Initial X-rays are frequently negative.

Carpal Tunnel vs. Cubital Tunnel

Carpal tunnel affects the median nerve: thumb, index, middle, and lateral ring finger. Cubital tunnel affects the ulnar nerve: ring and little finger, with inner elbow pain. These are frequently confused but require completely different treatment.
